Adult Day Services
Adult day programs provide older adults with a coordinated program of services in a community-based group setting. Services often follow either a social model that provides socialization and recreational type activities, or a medical model that focuses on skilled assessment, treatment and rehabilitation goals.

Nursing Homes
Also known as Skilled Nursing Facilities (SNF), Nursing Homes provide 24-hour medical and social services to frail elderly and chronically disabled individuals, and those in need of rehabilitation. All nursing homes provide inpatient rehabilitation. These residential facilities represent the most complex level of senior care on a short or long term basis. Many have Alzheimer's/Memory Care Units designed to serve individuals with memory impairments. Some offer short-term respite stays. A Patient Review Instrument (PRI) assessment and screening is required to authorize care in a nursing home. Rehabilitation services provide various combinations of professional therapies and medically-related disciplines to restore strength and improve living skills. Those listed here are for outpatient only, all nursing homes offer inpatient rehabilitation.

Respite Care can provide relief to a caregiver by providing a supportive environment for a loved one while the caregiver is vacationing, traveling on business, or during an illness. Respite Care can be provided in the home or in a facility.
