Together in Caring is a professional geriatric care management service that provides ongoing quality care management solutions for older adults, individuals living with Alzheimer's, dementia, or chronic illness, and their caregivers. Together in Caring serves residents residing in Monroe County, Wayne County, Ontario County, Yates County, Seneca County, and Livingston County.

Together in Caring was formed by Lifespan of Greater Rochester Inc., Entrusted Care, and Mark McDermott of Touching Hearts at Home in addition to a social work and nursing team possessing a longstanding track record of success supporting older adults in the Rochester and Finger Lakes area.
