Home Connection is a medical adult day program that provides quality care for seniors in a comfortable, stimulating environment. We provide family caregivers with a much-needed break while providing social interaction and health care for seniors, including those with disabilities. Offering flexible attendance, hot lunch meal and nutritious snacks, case management and social work counseling, physical, occupational and speech therapy, hygiene and bathing, podiatry visits, dental hygiene, hearing evaluations, lab and diagnostic studies, recreation therapy, exercise programs, field trips, and activities.
