Quaintly situated on the Highlands at Pittsford campus, Meadowbrook is a Home and Community Based Service that offers individuals the opportunity to socialize and remain active while ensuring their medical needs are being met. Meadowbrook provides daytime respite to families and care givers. Our services include a person centered care plan, nursing care, assistance with personal care and hygiene, recreational therapy, occupational and physical therapy, speech, nutritious meals and case management.
