The Leo Center for Caring is the first of it's kind in the area that is located within a skilled nursing facility. This 10-bed unit, located on the second floor of The Heritage, is dedicated solely to hospice and palliative care for community members to provide patients and their families with a home-like atmosphere of comfort and distinctive levels of care. St. Ann's Community currently provides palliative/hospice care to residents of St. Ann's Home and The Heritage in their rooms. Our medical staff consist of the Center's Medical Director, clinical nurse specialists, physical, occupational and speech therapists, medical social workers, dieticians, home health aides, and counseling professionals. Amenities include private rooms, TV, video, telephone and private baths. Family members are extended around-the-clock visiting privileges, food service, living/lounge area with a fully equipped kitchen, dining area and computer access. Overnight accommodations are also available to family members as well as support services and counseling. A partnership between St. Ann's Community and Visiting Nurse Service.
