Fidelis Care at Home is a program for those who are 18 or older, have Medicaid, and are eligible for nursing home level of care. Fidelis Care began its mission in 1993 under the auspices of the Catholic Medical Center of Brooklyn and Queens, Inc. and the Diocese of Brooklyn and now serves Medicaid beneficiaries and outreaches to the uninsured across the State.
