Supports people with physical disabilities in choosing and accomplishing successive, individualized life goals by providing a wide range of services to people of all ages in the Rochester area including: outpatient clinical, residential, educational and personal support services and permanent adaptations to living quarters which enhance quality of life. Certified Aging in Place specialists work closely with clinicians to assist resident in identifying, locating and acquiring the services necessary for them to remain as independent as possible in their own home.
