This NYS program serves individuals over age 60 who are not on Medicaid, and their caregivers. For those who need a little bit of assistance with activities of daily living, with the goal of staying safe in their own home. Subsidized or low-cost case management, in-home services (housekeeping, laundry, meal preparation, personal care), emergency response units, social day programs, home delivered meals and other supports when budget allows. A care plan is developed, based on individual need. Participants pay a cost share determined by their monthly income.
