Evaluation - PRI and Screen (Nursing Assessment)
PRI/Screen (Patient Review Instrument/Screen) is an assessment required in New York State to determine eligibility and appropriate level of care for those seeking skilled nursing home placement. It must be completed as part of the facility's application process by a specially trained registered nurse. (The Screen component may be completed by a specially trained social worker.) For patients in the hospital, the PRI/Screen will be completed by hospital staff. In the community, any of the agencies listed may provide the service.
