We are Aging Life Care Professionals/Geriatric Care Managers. Taking on the challenges of aging requires problem solvers who bring different perspectives and are willing to take risks. Aging Connections emerged out of a passion to support older adults and families. We help adults navigate through the chapters of aging by identifying resources and supports. We will navigate circumstances and transitions and our goal is to create empowered and supportive solutions.
